When she was a kid, Molly was a performer in the Bucks County Playhouse and also appeared later in productions at the Prince Music Theater and Arden Theatre Company. She earned her degree from Princeton University where she was an active member of the Triangle Club and choreographed and was in numerous productions. She made her Broadway debut as Little Red Riding Hood in Into the Woods in 2002. In 2004, she appeared in Fiddler on the Roof on Broadway as Bielke. Ephraim was a major character in 2003's TV film Hench at Home. She was in the 2008 film College Road Trip as Wendy. She also starred as Ali Rey in the movie Paranormal Activity 2 in 2010 and then reprised her part in 2014's Paranormal Activity: The Marked Ones. In 2014's Gravy in which she was the lead, she played Cricket. Ephraim has been the TV series Last Man Standing's Mandy Baxter since 2011. Ephraim has also appeared on episodes of the TV series Law & Order and Royal Pains.
